Patient Case: George Jones

George Jones is a 53-year-old male who comes to his primary care provider’s office with a recurring complaint of back pain for which he wants a prescription refilled. The nurse and nurse practitioner he sees are concerned about his lack of follow up with referrals to physical therapy and occupational therapy and about a possible substance use disorder. The clinicians in this and the subsequent videos are trying to guide George away from what might be a developing opioid disorder and toward an interdisciplinary chronic pain management model. There are three videos – one where George visits his primary care clinic to get an early refill of Vicodin, and two versions of George going through an intake evaluation in a team based chronic pain management program. Version 1 demonstrates flawed team interaction and the Version 2 video presents an idealized team interaction.
